This verification may be needed for things like (among):

This form may not be used for:

• Requesting transcripts

This form requires the following information:

• Student's full legal name
• Student's date of birth
• The academic year that the student enrolled at Sora Schools (your first year at Sora)
• The reason you're requesting this form
